An email was sent On Aug 3 with details of Dr. Fox’s retirement. You are required to complete new roster forms with Dr. Rosas if you wish to stay with the practice. The forms from the Ministry are now in and have been sent by email to most of you. If you did not receive one, please contact us and we will arrange to get one to you. You have 6 months from October 1st to complete the form and return to us.
